A couple of things, I've been reading this spat and Macnamara answered a question then it's been jumped on by repetitive questions, he never said he would be a leader just answered what his solution might be to the overall problem. You might not have liked his response but it's what he thinks is best. I thought this was a thread about covid not arguing over the use of smart phones. Unfortunately letting the chips fall is going to change nothing as the chips are manipulated to fall where they want them to fall. If you take the covid figures for vaccines as true they have the masses sewn up and will do whatever they like without push back. (95% around me have had vaccines so I see some truth in the percentages being bound about) at least by not using smart tech you taking provisions against harmful blasts. You would not know that this is the case with all the stores in compliance with the so called mandatory masks, social distancing and following guidelines. But the UK is worse, even worse than the US and Canada. In all three countries both doctors and nurses have had serious pay cuts and increased acuity and patient numbers along side cap staffing ratios. Which has gotten worse every year since the 1980s. The system completely runs off of the slavery and exploition of licensed staff. My hospital has cut down nurse staffing levels so severely that each nurse has 9 patients and no help, on her own for 12 hours. These are critical and complicated patients that need one to one monitoring. It forces her to stay past her shift another 4 hours to chart. A grand total of 15 or 16 hours with no break. 6 of my 9 patients last night were 300lbs plus and dead weight, constantly pissing themselves or trying to crawl out of bed and fall. Each one on the call bell 8 or 9 times an hour. The other three were critically ill, on complex time consuming drips and interventions. Management has also taken away aides, phlebotomy, cleaning staff and lab techs, and outsourced catering to a horrible company that dumps out trays and then takes them away before a nurse can even get around to all her people and make sure they eat. I hurt today. Almost every nurse we have hired over the last three years has quit after a month on the job, calling and crying and saying I will not come back. Occasionally, they will last for a year. The job market is bad for nurses except for jobs with these insane conditions. Not only is it physically and mentally demanding but the patients and families are abusive and demanding with no respect or comprehension of the fact that they are sharing one RN with multiple other people. For the nurse, the fact that she could get held responsible for someone's death simply because she cannot be 10 places at once is a very real fear. It is physically impossible to stay on top of second level tasks that prevent suffering, and all that gets combined with abusive families, patients, etc. No one abuses the managers who cut down the staffing levels, or medicare for refusing to reimburse for safe nurse staffing levels. Management cuts the staffing levels, but the public goes gunning for the throat of the overstretched nurse when their loved one is suffering and she cannot get into his room because her other patients are critically ill and need immediate interventions. Management is off the hook for the decisions they make. Hospitals only hire to replacing nurses who leave, never to increase the staffing numbers. Colleges are churning out lots of new RNs a-plenty but once they get a taste of hell on the frontlines they run screaming from hospitals. It is a manufactured "shortage"so the powers that be can profit and harm patients. There is no real nursing shortage. Nurses would stay in the hospitals if the ratios were safe. In 2018 20 nurses quit and they replaced them with 16. 30 quit and they replace them with 12. On and on. They are always hiring but never increasing the staffing levels, making the job too brutal to continue in. I was told they want to run every last American nurse out of the job with poor and abusive conditions. Not only are the staffing ratios abusive and dangerous, but the public is encouraged to lash out at the nurse and not the policymakers. They want to replace American nurses with foreign ones who will not question dangerous practices, who will work for peanuts, and will be trapped in the job because it is tied to their immigration visa. Last night we reported our hospital to patient safety organizations and the DoH. But that has been done 1000x before and they don't respond because they have their hands in the same cookie jar as management and the feds. Management/ CFOs have SHUT OFF COMPLETELY from communicating with us. They know they can do what they want and that the public will attack the nurse. They could stretch one nurse out between 100 patients with no help. That nurse will get accused of intentional neglect and laziness. The doctors only diagnose and prescribe, they don't get involved with anything else. They just shrug their shoulders and say it's not their problem. They are another group of people who sit and watch management cut off the staffing levels and then lash out at the nurse. They only have to develop a correct diagnosis and an order set. It is not their problem if that order set requires the nurse to be in that patient's room 60 minutes out of every hour and she has no help for her other 8 people who are now being harmed because she is away from them. I had 8 heavy, complicated patients last night with no help and the 9th that they sent me on top of all that should have been in an ICU with a one to one nurse. You are right. It has nothing to do with covid. It has been going on for years but they are using covid as an excuse. Not only is everything I described happening but they are cutting supplies. Last night all nine of my patients needed labs (bloodwork-which on it's own is impossibly time consuming just to obtain the samples) and we were out most of the collection tubes we needed. Plus the lab techs and aids have been taken away, the pneumatic tube system was down.....I lost two hours searching for supplies and then had to put my license at risk by leaving my patients to walk the samples to the lab. Otherwise, those tests would not have been run and something like that is more likely to kill a patient than a med error. Labs are extremely important. Abandoning my patients was bad, they could have been hurt badly because I was off the floor, but if an electrolyte imbalance did not get caught in time because I didn't walk those labs down I would have got the blame for that death. Almost all support staff for that kind of thing (courier) has been taken away. Too add insult to injury, they now have the unit secretary covering 4 or 5 units. This is overwhelming for her so she turns off her phone and diverts all calls to the nurse. That means that while your nurse is trying to titrate your insulin gtt, monitor your cardiac tele, obtain your blood, and look at your orders and clinical picture she is getting 10 calls a minute from irate families of other patients plus she has 8 other patients screaming for her. The secretary would have previously tried to buffer those phone calls at least. I don't think there is a massive difference between Trump or Biden in who they discriminate against either. They both collude with temple building masons, and they see us all as people that need to be screwed over so they can have their temple, but I can't really be arsed to keep getting into that.
