Icke describes the system using a pyramid - only the few at the top are in the know and the rest of us are fiercely compartmentalised to keep us in the dark. Workers go on doing their job without really realising what they are part of.
Workers further on up who are in the 'know' a little more are blackmailed, threatened to keep them in line. So how many are controlled like this? For instance have you watched the film SAW? Good people convinced to do terrible things because they have been blackmailed or threatened. Many people were controlled like this by one person. I've always wondered whether it could be possible that there is actually a single person at the top of the pyramid.
